Refer a Patient

Dr. Euler is currently taking referrals for his online Cognitive Behavioral Therapy groups for depression and anxiety. These groups are 12 weeks in duration and offered virtually via Zoom. Patients must have a high-speed internet connection, and a working computer or device (e.g., phone, tablet) that allows for video meetings in order to participate. Patients must also have OHIP coverage and must be in Ontario at the time of all virtual meetings.

If you are interested in participating, please have you family doctor of psychiatrist complete and fax the referral form below.

Once you have been referred, please see the “Newly Referred Patients” section along to the top of the website to learn more about wait times and when you will hear from Dr. Euler in order to book your assessment.
